This study explores the social determinants of health contributing to disparities in isolated communities in Puerto Rico. A community forum was conducted with the objectives of identifying key health-related issues and developing strategies to address these priorities. Using the triangulation method for qualitative data, the study identified five main health priorities: access to health services, issues affecting older adults, access to basic services, preparedness for future disasters/emergencies, and COVID-19 vaccination. The forum resulted in the development of fifteen strategies, focusing on preparedness for natural disasters and COVID-19 vaccination. These strategies aim to reduce health disparities in these isolated communities, which face a complex intersection of health-related risks and outcomes.
